I'm going to nominate The Legends of Romia: Kamala's Tomb by Blaman. It's not THAT old(it's for a 2.5 beta, albeit an early one), but I'm pretty sure it didn't generate too much buzz and may have been lost in a swarm of other submissions near the same time.
