heres what I got so far

You'll hafta make all the tiles of destroyed stuff. Junkyard cars... broken tombstones... trash and shit everywhere... burnt bushes... roadkill layin around... drunks everywhere... For Kakariko Village, you could always make that of homeless people living in boxes n stuff

Then there's always the rich drug dealin niggas, maybe use their cribs as dungeons

I think it would be better to edit the item sprites rather than use the regular ones.

Bag of Cocaine = medicine or somethin
Swords = baseball bats or somethin
The meat = can of SPAM
bombs = grenades
The letter you give to old lady = Porn Magazine
The lady you give the Porn Magazine to = Hooker!!
the guy you get the Porn Magazine from = Hookers Pimp!!
